I am a registered Occupational Therapist with over 25 years of experience, holding an Honours Bachelor of Arts in Psychology from Western University and a Bachelor of Science in Occupational Therapy from the University of Toronto. I am the owner of Lori G. Eisen Occupational Therapy Services and provide occupational therapy assessment and treatment, and case management and life care planning services across both the community and private sectors, including auto insurance (SABS & Tort), medical malpractice, elder care and long-term disability. My clinical expertise includes working with individuals who have sustained spinal cord injuries, traumatic brain injuries, cognitive impairment, amputations, complex orthopedic injuries and multiple traumas. I also have extensive experience in hospital discharge planning, in-home and worksite assessments, functional restoration programs, Assessment of Attendant Care Needs, catastrophic determination assessments, home modification, and working as part of a multidisciplinary team. I work extensively with older adults as a geriatric care manager and provide Geriatric Legal/Functional Living Assessments in estate litigation. I have authored many Loss of Service and Future Cost of Care reports/Life Care Plans which are used in mediation, arbitration, settlement discussions, litigation and rehabilitation planning for both plaintiff and defence referral sources. I am a Certified Catastrophic Impairment Assessor based on the AMA Guides to the Evaluation of Permanent Impairment, 4th Edition. I pride myself on possessing excellent communication and interpersonal skills, as well as being a flexible, motivated and responsible professional. My report format is organized, and all recommendations are thoroughly researched and justified.
