Persons certified in any of the 7 certification credentials offered by the ICHCC will find all forms necessary for recertification and for documenting CEU's. The non-credentialed health care professional will find applications for the Certified Life Care Planner, Canadian Certified Life Care Planner, the Medicare Set-aside Certified Consultant, the Certified Geriatric Care Manager, the Certified Healthcare Risk Manager, the Certified Life Care Manager, and the Certified Disability Evaluator. Training organizations wishing to apply for CEU coverage by the ICHCC will find their form for CEU application as well.
